My child has my permission to participate in rehearsals for HopeBox Benefit Concert. Attendance is very important to finish the concert successfully. I agree to encourage my child to attend every rehearsal unless compelling reasons arise.

I agree to waive all claims against HopeBox for any injury or property damage that my child may incur. I waive any claim, suit, or other legal action against HopeBox in the event that my child suffers any injuries as a result of his/her participation in activities related to the concert.

I give permission to publish, distribute and/or display video and photographic images of my child for archival and publicity purposes.

I acknowledge and agree that HopeBox club ("the Club") utilizes third-party software and services, including but not limited to zeffy and groups.io, in its operations.

While the Club strives to select reliable and secure software, the Club cannot assume any responsibility or liability for any issues, errors, or malfunctions that may arise from the use of these third-party software tools. By registering as a member of the Club, I acknowledge and agree that the Club shall not be held liable for any damages, losses, or other consequences resulting from the performance or failure of these third-party software tools. My use of club services constitutes acceptance of this disclaimer.
