HopeBox would like to thank the students who volunteered at HopeBox for one year (July 1, 202x – June 30, 202x). Students who have done at least 50% of their volunteer hours at HopeBox during the period July 1, 202x – June 30, 202x can apply for a PVSA at HopeBox by adding up their volunteer hours from other volunteer institutions (in addition to their HopeBox hours). Please check the PVSA eligibility requirements below, and if you would like to apply for PVSA at HopeBox, please fill out the required documents below and send them to Eileen (eileenlee@hop3box.org) by July 15th.
Eligibility Requirements:
- U.S. citizen or green card holder
- Volunteer hours from July 1, 202x – June 30, 202x
- At least 50% of volunteer hours must be from hop3box
- Hours Required to Earn Awards in Each Age Group (Red indicates # of hop3box hours needed)

Required documents:
- Your legal full name and birthdate
- A copy of US passport or Green Card to prove your residency status
- Verified volunteer hour certification from all other PVSA organizations which log your volunteer hours performed.
- A completed “Hop3box President’s Volunteer Service Award Application” – See attached PDF form.
